Comments:
At time of inspection no violations noted. Food and refrigeration units were well within required range. Facility appeared clean and well managed. Food gloves, date marking sanitizer test kit and hair restraints noted.

At the time of the inspection no violations were observed, nor was there any food preparation occurring. Use of food gloves, hair restraints, date marking and sanitizer test kit noted. Smoothie dispenser cleaned weekly. Temperature log maintained daily

At time of the inspection no violations were noted. Observed food gloves, hair restraints, date marking and sanitizer test kit. Fife extinguisher date up to date.

Violations: - 1570 - Corrected During Inspection Bevair reach-in make table unit (back room) was observed in a state of disrepair.
Repair the unit to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
- 2200 - Critical Repeat An air gap between the water supply outlet and the flood rim level of the mop sink is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.
Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.
Comments:
Remember to wash hands and wear hair restaints. Observed santizer kit and date marking. overall facility looked clean and tidy.

Violations: - 2000 - Boxes were found stored on the floor.
Store everything in a clean, dry location where not exposed to splash, dust or other contamination at least 6 inches above the floor.
- 2020 - Utensils were found handled, displayed or dispensed with the food or lip-contact surface facing upward.
Store utensils with food or lip-contact surface facing downward to prevent contamination prior to use.
- 2200 - Critical An air gap between the water supply outlet and the flood rim level mop sink is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.

Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
Comments:
Overall, facility is in good order. Remind employees to wear hair restraints. Suggest a cover for hot dog cooker. Thermometers are checked for accuracy daily. Observed sanitizer test kits, the use of gloves, and date marking.

Comments:
Ice scoop must have handle up. Hand sink hot water, ensure temp is at 110F Kitchen - serve area Bev-Air 0F At time of inspection, past violations were corrected.
June 05, 2007 (Pre-Opening)
Comments:
Construction Inspection: estimated date of opening 7/29/07, but the food service will likely open around 6/22/07 to serve the workers. Store manager is C. Poole. Call June 21st to schedule pre-opening. Ask for Werner, EHS Supervisor. Mail $40 annual permit fee to the Warren Co. Health Department. Hose at mop sink must not extend below the rim of the sink (corrected) Elevate Shelf - next to pizza oven (corrected) Add cover to hot dog cooker. Add internal thermometers to refrigerators that don't yet have one.
