Comments:
Follow up evaluation. All violations corrected with exception of Ansul system. Per PIc Fire X came out on 3/23/09 to check. Will need to replace system. Fire extinguishers updated and will use these until Ansul system fixed. Disclosure added to Reminder statement on Breakfast menu. Need copy of work order for Ansul system from Fire X and payment recieved by 3/27/09.

Violation: 3180 - The following physical structures noted in need of cleaning:-behind grill- mop bucket area- floors behind freezers in basement
All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
Comments:
Ansul system observed with 2005 tag. Needs to be inspected every year. Call asap. Fire extinguishers checked 02/09. Good use of date marking, glove use. Discussed employee health, cooling and reheating methods and internal temperatures. Handouts given to same. Disclosure needed on breakfast menu. * by items or all eggs then at bottom in frton of consume raw statement, put these items or all egg items may be ordered undercooked. EHS to follow up on 3/25/09 to check on corrections.

December 22, 2008 (Risk Factor Assessment)

Comments:
Buttermilks unopened observed with 12/18/08 date. NEed to use soon. Whole milk in display case opened and observed with 12/19/08 date. use soon. Food contact surfaces observed clean. Date marking observed. Reminder to wash hands and wear gloves with RTE foods. REminder to use 50-100ppm bleach as sanitizer. Consumer advisory observed not on menu. Need for egg products. Disclosure and reminder language discussed and what needs to be on menu. Need by next inspection.

September 10, 2008 (Risk Factor Assessment)


Violation: 0820 A 2 - Corrected During Inspection Critical Sausage patties cold holding at improper temperatures.
Cold hold potentially hazardous food at 41F or below to inhibit the growth of harmful bacteria. (patties at 63 degrees x 2 days) Food to be discarded if >41 degrees for >4 hours.
Comments:
Food observed stored < 6 inches off floor(onion). PIC moved to shelf. Discussed new reg updates, cooling methods, handwashing and glove use policy. ADvised PIc of observances made for correction for next inspection.

Comments:
All violations corrected. Permit renewed.

February 11, 2008 (Routine)


Violations: Comments:
Commercially processed buttermilk observed expired beyond use by date. Since date is for quality purposes and not food safety advised PIC to use as soon as possible. Reminder to keep ice scoop with handle up in ice machine. Discussed date marking of frozen phf products and using handsink for handwashing and not 3 compartment sink. Good use of hair restraint and glove use. Discussed CDC Risk factors, and updated food regs. Handouts given on updated food regs and training schedule. EHS to follow on March 17th for correction of violations. EHS left Demo of Knowledge for PIC to complete by follow up inspection.
