| This visit was made to conduct a routine food safety evaluation. The Person in Charge was very helpful during the visit. Thank you. The following issue requires attention: 1. Please obtain test strips for the sanitizing solution being used. The concentration of quaternary ammonium should be between 100 - 400 ppm. Note: - Provided an updated Employee Health Policy Poster and discussed its requirements - The facility is implementing active pest control methods and there was no evidence of pests at the time of the visit. Please continue to clean frequently. The dry storage room was very well organized and clean at time of visit. Thank you - A handwashing sticker was provided to the Person in Charge. Please discuss with the church about placing them in the bathrooms. *Repeat Observations are Subject to Civil Penalty

| This visit was made to conduct a change of ownership inspection. The following items require correction within 30 days: 1) Reinstall the mop rack (it has fallen from the wall). Note- Hot water deficiency letter signed by establishment. Approved for change of owner. Recommend issuance of Alexandria Health Department (AHD) permit. This report shall serve as your eprmit to operate unitl you offficial permit is received. Any changes to floor plan, equipment, menus/special processes shall be pre-approved by AHD.
