What would be a reasonable commute outside of the perimeter? 1/2 hour to 40 mins max, commuting to Midtown.

Smyrna, in Southeastern Cobb County, is most likely one of the absolute few places (if not the only place) outside of the I-285 Perimeter where that type of relatively short commute is even possible during rush hour.

You could find a place in Dunwoody, Sandy Springs or Roswell close to MARTA. From the North Springs station it's 23 minutes to the Midtown station by rail.

No. People should avoid Smyrna now since that new baseball stadium is being built. Afternoon rush hour is going to be horrible for 70+ days a year

Does it have to be OTP?

Vinings or Brookhaven would work. Though I'd personally prefer Vinings, it is hard to tell what's going to happen to traffic after they start building the new stadium in Smyrna in "Cobb's Platinum Triangle".

Outside the Perimeter, your options are few. Smyrna would be one and Lithia Springs and Douglasville out to the west you can do in 40 minutes. If you live in Dunwoody or Sandy Springs, the train will have you there in about 20 minutes but a car commute would be a killer down 400 and the Connector.

It can make a difference which part of midtown. . . If you are out toward the westside, then Smyrna. . . What is the rationale for being OTP? Is it for better schools? big enough yard for a dog? lower taxes? More information and we could help you better.

Excellent points about Dunwoody and Sandy Springs having good access to Midtown by way of the MARTA Red Line heavy rail train.

But rush hour commutes between Lithia Springs and Douglasville can be very-challenging at times because of the very-heavy traffic and sometimes outright gridlock (depending on traffic and/or weather conditions) on I-20 West between Douglasville and the I-285 Perimeter during morning and evening rush hours.

Because of the very-heavy rush hour traffic on I-20 West outside of the I-285 Perimeter (heavy enough traffic that the state has expressed an intense desire to rebuild the I-20/I-285 West interchange and place truck lanes and/or reversible toll lanes on I-20 West between I-285 and GA Hwy 6 Thornton Road), and because of the very-heavy traffic on the I-75/I-85 Downtown Connector between Midtown and I-20 during morning and afternoon/evening rush hours, I-20 West corridor areas like Lithia Springs and Douglasville likely would not be very-good options for the OP to consider for a 30-40 minute max rush hour commute between someplace outside of the I-285 Perimeter and Midtown.
