I'm also one of the many that moved from IL to AZ. The highways in IL - top notch for the most part. The tollways around Chicago that were rebuilt a few years ago were done right - thick concrete, very smooth, 4 lanes in each direction. But when you get off the highways, the roads are terrible. I bent 6 rims in three years driving on US-41. That's not even saying anything about the potholes in winter. Just bad... It was a revelation when I got out to AZ. My only complaint is that there are large seams in the road. Still, very smooth and quiet. And I won't have to worry about replacing control arms and straightening wheels every few years.
The best I've experienced has got to be North Carolina, Virginia and Maryland.
The worse, Pennsylvania, Indiana (especially Indianapolis area), Illinois (especially Chicago area), Michigan (especially Deeeetroit area) and Washington DC.
Michigan has the higest weight laws in the country for semi trucks they can carry up to 160,000 lbs. That's one reason the roads are tore up. Lots of steel haulers for the auto companies.
I haven't been to West Virginia in many years but when I was driving over the road they had the WORST two lane roads in the country. Their Interstates were OK but get off of them. No way.

I used to do a lot of traveling, and I agree with every state on your list of "bad roads states". All of them have awful roads.
