My experience is Midwest drivers are most cautious, but I wouldn't say best drivers - just most risk adverse and least assertive on the road. In a city, that is a little dangerous - your decisions have to be quick and decisive, merging and exiting have to be fast, and you have to learn to change lanes by nudging into where you want to go at appropriate times. You can't leave 250 feet in front of you while driving in traffic to keep a safe distance at 65/mph, it just doesn't work.

So with the sheer amount of people competing for limiting roadways around NYC, we probably aren't considered the most polite and courteous drivers, but we're not the worst. Smaller cities fare worse. In rain or snow, Northeast drivers do all right while drivers in the South pull over and take out their phones to take video. Drivers out west drive well on the highway, but in cities they're a pain at being ready for left arrows and stopping too soon on yellows.

Here's some data on how states/cities do:

States with the most fatal car accidents in 2010, according to the National Highway Traffic Safety Administration:

The worst states in the country, per capita, for traffic fatalities are Wyoming, Mississippi, Montana, and North Dakota.

Allstate's "Best Drivers Report" survey in 2011 listed 200 cities by the number of traffic accident claims, and the following cities (over 1 million) as having the fewest number of years on average between collisions:

No. 182 Los Angeles 6.6
No. 171 New York 7.3
No. 167 Dallas 7.5
No. 157 Chicago 7.7
No. 155 Houston 7.8
No. 142 San Antonio 8.1

Their results for 2012 listed Sioux Falls as the safest city in America with its average 13.8 years between collisions. For most of the rest of cities in 2012 (excluding Massachusetts which did not have coverage through Allstate, and those cities that Allstate did not put out a press release for) they fared as such:
