Requests to disassociate name from street-level assessment address (Boise: county, working)

Hello. I have tried to disassociate my name from the street-level assessment address several times. Each time the request is rejected saying "wrong city/county". However, I am entering the correct city and county.

The address is on this page:

The city for all addresses on this page is Boise.
The county for all addresses on this page is Ada.

I have tried the following combinations of city and county:
Boise, Ada
Boise City, Ada
Boise, Ada County
Boise City, Ada County

Why isn't this working?
