Haven't upgraded in two years, and now I find out Verizon is charging a $30 "Up Grade Fee". What the heck is that all about; I know, just another way to get $30 more out of us consumers.
Do all the major carriers charge an "Upgrade Fee"?
Yes they do if you get a phone at a subsidized price. Been this way for as long as I remember with all carriers. In some cases the fee can be waived but not regularly.
Officially, the carriers have always made the upgrade fee a requirement when upgrading, but often times CS or sales reps waived them as a courtesy. Not so much today.

They also "used to" give you a discount on the subsidized price when you upgraded to a new phone with a new contract. Not so much today.
That must be something new or only in your district because once you're able to upgrade you can do so without having to pay a fee. There are only certain phones that you can get though so if you decide on another phone that's not available for your upgrade you'll have to pay the extra fee.
