my guess is that she was walking along staring at her screen composing a series of tweets about her victorious conquest of angry birds while listening to Justin Bieber and she walked into an open electical transformer...death by electrocution.

"Electrocuted by iPhone" is not the same as electrocuted by the energy delivered by a AC power outlet - which is apparently what happened.
The thread is called electrocuted by iPhone because the iPhone is the device in question. The AC power outlet worked correctly delivering electricity from the wall to the charger. The charger(maybe the charger delivered too much we don't know) then delivered power to the phone as intended. The Iphone did not function correctly in delivering the electricity to the user causing his or her death.

We can all speculate on what caused the electrocution but until the final story is told will we won't know anything for certain.

Ohm my god, I'm getting a charge out of all these comments. Some posters Flip-Flop or Oscillate back and forth on their current opinions - too much feedback from one post. Watt's up with all these comments? I don't have the capacitance for much more - don't want to show any resistance or I'll get hot and have to turn on the AC. My head Hertz from all the dumb jokes too. I tried to run away from my computer but I was slowed down as I tried to crossover the street on my cycle because I had to let the band pass by.

Seriously, from a technical standpoint chargers are supposed to supply isolated low voltage power that is limited in amperage. That isolation would have had to fail, and a circuit be made through the body of the person to ground. The fault (yes, that is a proper electrical term) would have been in the charger unless one side of the phone case was at one potential and the other side at a different one, so that a circuit was made between ear and hand.
