I have the iPhone 4S:

Whenever I manually shut off the device (i.e clicking the button at the top, and then sliding to power off) the phone will automatically turn itself back on. Has this happened to anyone else??

Somehow, I don't think it is actually being turned off completely.

Is it plugged in?

It's been a long time since I've turned off an iOS device, but I vaguely recall many years ago that powering off my iPhone while it was connected to a charger would cause it to reboot.

Did you do a software update recently?
My iPhone 4S did that after a software update and Apple ended up having to give me a brand new phone for free because their update killed my phone.
The biggest issue I had was the phone would turn itself on and off a dozen times before it would either stay off or stay on.

Google it on your specific model and make and you find info including some techs who have seen it as to what they found.
