Business plans include it, I didn't think personal ones did...
We have had Verizon for more than 10 years and we both have iPhone 4S, there has never been an issue tethering to the iPad, iPhone, iPad Mini, desktop computer or laptops and we have never been charged extra for tethering and we do not have and never had a business account, only personal.
except for when apple decides to create a video streaming site and remove the netflix/crackle/hulu/amazon/... app from the apple app store.
i think tethering/hotspot is no longer a big deal on most cell-fones. many cell-fones put arbitrary restriction on it since 4g was a fairly new resource and cell-fone companies put pressure on manufacturers to limit access to it so they can sell that functionality as a premium and limit internet throughput consumption.
these days most fones are fairly easy to root and bypass that restriction and most carriers no longer care.
I don't recall them ever removing them from the App Store. The whole Maps thing was based around the fact that Google did not give their iPhone app turn-by-turn directions and their contract had run out as the default Maps app for the iPhone. Google Maps was still available for download in the app store if users preferred to use it. I've never heard anything about not being able to use Youtube. Youtube would have a revenue collapse if iPhone users couldn't use it.

I don't think the issue is Apple removing applications as much as it is them not allowing legitimate applications. The Google Chrome Browser is a good example. Because Apple has a competing product, they don't allow other browsers.
Depend on the carrier. With ATT and Verizon, no extra charge. And for the third time the iPhone does tethering. Been the case for years and is real easy to do.
It depends on the plan. A grandfathered unlimited plan on ATT without the tethering pacakge can not tether without changing the plan.
