Didn't you get the Rules of the Road book in Driver's Ed? It says what you need to bring. You can also find this information here (Acceptable Identification (http://www.cyberdriveillinois.com/departments/drivers/drivers_license/acceptable_id.html - broken link)).

If possible, go to the Woodstock location. It is much smaller and you will be in and out. Lombard is a nice town, but the DMV is a nightmare! You will be waiting most of your adult life in one line only to be told to go into another line. You will also encounter some of the most unfriendly people (employees) that you will ever come accross.
