Try the Urban Vegan on Montrose and Ashland (admittedly not sure what neighborhood that is - Uptown, maybe?). It's a vegan pan-Asian restaurant with emphasis on Thai.

All I can tell you is that I recall going to Blind Faith Cafe on Lincoln Avenue when I first moved to Chicago back in the mid 1990s. Unless that was a second location, which it could have been.

That's the part of Ravenswood that is in Uptown.

Must have been. It was only open for a year according to this suntimes article.

There's a Veggi Grill (Vegan Fast Food) coming to Lincoln Park this fall. It's the first one to open outside of the west coast.
