Would El Paso TX qualify as a major city? It's in the top 25, and when you combine its population with neighboring Juarez Mexico, there are about 2 million people in proximity to the airport. Yet, it's a single terminal airport. The last time I was there, on a shuttle bus, right before getting dropped off, the bus driver casually mentions that a bull snake is slithering across the road from the parking area through the main loop. I had to do a double take, but sure enough, there's a huge snake approaching the curbside baggage check in next to American Airlines.
El Paso is a medium sized city with a pretty small airport, but I think it is laid out and operated pretty well. It is also pretty clean and attractive, though heavily southwestern themed.
On the other hand, I know of many in the northern Chicago suburbs who like to fly Milwaukee to avoid the crowds/hassle/delays of O'Hare!
Yeah, and Rockford (around 400,000 in the area) and Gary also have airports that are trying to draw metro Chicago people away from Midway or O'hare to use their airports.
Rockford even calls their airport the Chicago/Rockford Airport. They're probably 30-40 miles northeast of the suburbs up there.
I think in the future there will be more to choose from depending on what side of the metro you live on.
San Diego does suffer from being in the shadow of L.A. airports somewhat. But what an interesting approach flying into SAN. The angle flights come in at, and the closeness to buildings is a little disconcerting.

Reagan National, yes. However as mentioned above it did build a big new concourse, I think in the 90's. But its original old terminal is still in use.
In general I think cities with two or more airports generally tend to have smaller facilities. Exceptions, Los Angeles and Chicago. But in New York, LaGuardia is quite small.
I agree Kansas City's airport is not user-friendly. There are plans to redo the entire airport, but I believe they are on hold.
O'Hare in Chicago is certainly not small, but it still has 1961 terminals in place that show their age.

7 1/2 years since this quote the Kansas City's airport's future is still on hold. I doubt that any in our lifetime will see a change. sigh.....
I remember flying into Detroit City Airport on Southwest Airlines in 1994. That one wins this contest hands down!
That airport hasn't been used for commercial passenger traffic since 1995. Would it even count?
