Wilmington as NC's largest coastal city also has the largest hospital of any coastal city in the state. It is far enough south that it will not get a real winter. Compared to New England, even Asheville (in what passes for mountains in NC) does not have a real winter. There are other cities along the coast with good hospitals, Morehead City comes to mind most easily, smaller, closer knit, friendly. Doesn't have all the amenities that come with a larger metropolitan area, but, is friendly, comfortable place. Either would be a good choice.
