I have seen my college transcripts and may have looked at my HS transcript once. It looked pretty factual...just a list of classes and grades, and maybe class rank. I wonder how a narrative of what went down would be accurately depicted on a HS transcript.
Sure, taking mental health issues into account is increasingly important for any school. The OP, and applicant, needs to seek help and find ways to cope so these "meltdowns" don't happen again and thereby increase his/her chances of life success.

Guidance counselors are an additional mandatory evaluator and they absolutely can say negative things.

The OP mentions IEP, therefore he must be a special ed student. He might be trying to apply for one of those "remedial" type of programs at a CC. The incident might be noted on the IEP or other special ed documents. It probably won't be on the actual HS transcript.
