If the OP doesn't drive, he might as well forget Colorado. Away any distance from the metro areas, driving becomes about the only option to get anywhere. Greyhound doesn't even serve many areas of the state, anymore. Amtrak only has two routes in Colorado, with just a handful of stops on both. The route of the Southwest Chief in southeast Colorado is in serious jeopardy of being removed from Colorado altogether, unless Kansas, Colorado, and New Mexico agree to pony up state funding to upgrade the track on the route. Personally, I think that his combination of medical issues and desire to live in a remote area are not very compatible.

He might look at one of the smaller communities in Washington state that is served by Amtrak's Cascades service. Living costs will be higher there, but access to everything is also much easier.

Transportation isn't the best in rural Colorado, true. But there are some options. The Black Hills Stage Line still runs through central Colorado, even stopping in Moffat and Cotopaxi along with Salida, Buena Vista, Pueblo and Denver. Those are just a few of the communities it serves.

There are other options as well. Say you live in Victor. Take the Cripple Creek/Victor shuttle between the towns and the catch the Ramblin' Express casino shuttle from Cripple Creek to Colorado Springs. From there, Greyhound will take you to points beyond.

If public transporation is important, you have to be really careful about where you live in rural Colorado. But there are remote options with ways to get out.
