She initially went to the hospital for breathing problems, then dies from a blood clot in her lung.

What, exactly, did the doctors at the hospital do? They obviously did not find what was causing her breathing problem.

I think this is just so sad the entire way around. The woman knew something was terribly wrong, and was at Hospital. The police are not at fault in any way, they were only responding to a call. The hospital is at fault, if there is any at all. Of course, this is just an opinion, and everyone has one of those...
I feel so terrible for the lady's family.

The ER notes would and will tell a lot. That is why I question any ME's report to this point.
