And for threatening a flight attendant, the police would have been called, you would have been escorted or 'taken' off the plane, promptly charged and spent the night in jail.

You're not. You are free to drive to your destination.

Would make sense to ban all non service animals inside the passenger cabin, then mistakes like this wouldn't happen.

Pets have safely traveled in carriers on airplanes for a long time. Perhaps it would make sense to train flight attendants about how to manage them.

You aren't. You may choose not to fly.
The people are arguing that the convenience of the pet or the pet owner overrides that of everyone else. That is not the right answer. See Animal Rights v. Peoples' Rights; People Trump Animals. Excerpt:

This is a particular problem because ESA's don't require or have any special certification. For a dog to become a true service animal extensive training and testing are required. Most dogs do not pass "seeing eye" training for example. They have a somewhat easier time becoming a therapy dog but even there the population is limited.

By contrast, to some extent most pets are "ESA's." Being greeted by a wagging tail on coming home from school or work melts most people's hearts.

In my view, humans are "keystone species" and must make the decision for animals, not the other way around. While animals must be protected from unnecessary cruelty people's rights must prevail.

How come the flight crews are not informed of passenger with pet boarding? Poor dog and stupid owner.

Again not a vacation. They were moving to a new home I believe. .

What sort of vacation would be possible for one adult carting around a child, an infant and a puppy??

In United's limited defense, sometimes the flight crew might not have time to be informed or are perhaps ill-trained. However, that doesn't excuse this tragic event.

The receive a manifest before the flight, letting them know how many souls are on board. They are also alerted to the seat locations of passengers with accessibility issues and (on longer flights where meals will be served) special dietary requests. It doesn't seem like it would be that difficult for the same manifest to report the number of paid pets on board and their seat location.

At least one witness heard the dog's owner telling the flight attendant that there was a dog in the carrier.
