Average price for new retainer? (dentist, cost, permanent, mouth)

I had braces (twice). I lost the mold the last dentist made of my mouth. I soak the retainers every day in denture cleaner tablets, but there is some plaque build up on one of the them. Plus there is a crack on one of them. Has anyone had new retainers made, and if so, how much did it cost?

I need to get my retainer placed back into my mouth. I have a permanent one in back and was quoted $200.

I suppose they made another mold of your mouth to make the new retainer?
