If you see a patient is dropped off by someone (friend, family member) to your clinic and later that someone comes to pick them up and asks you if they are still here, and there is no ROI on file for that person... would telling them whether or not the patient is there be a HIPAA violation?

How would that be a HIPAA violation? It has nothing to do w/ medical information

I'm in healthcare compliance. No, if it is simply what you are describing, it is no violation of privacy rules.

Some have misinterpreted that HIPAA means "no communication, no how, no way" but that's not the case.

The only time I would not disclose such info is if the patient/client has been abused or threatened by the person asking, or if the patient specifically asked us not to share any info with a particular person. But if it's the person picking them up, that's not the case.
