I'm a huge magic fan and used to want to be a magician when I was a little kid. There are actually a few ways to pull of this trick. I'm not sure which method he is using.
He most likely had a deck like this or one similar. It is of course possible to do it without a gimmicked deck, but only if you're holding the deck. You can't really use sleight of hand if your hands are away from the deck.
People who are impressed by David Blaine should check out Daniel Garcia.
When she places the card on the top, he pretends to cut the deck but he keeps her card on top. Then when he places it in the trick box it slides up the top card, her card. He is good but that is a pretty weak trick for someone like him to do. That is a trick you would expect to see at a party that someone bought at a magic shop.
I used to like David Blaine until he almost killed himself inside an ice block for one of his stunts, a few years ago. That's when I knew he had gone too far and needed to slow down and think things more thoroughly, instead of seeking fame at whatever the cost.
