If your sister is really into horses, the horseshoe one would be okay.
This. I think the horseshoe is not nearly as popular, thus the difference in price Also, with the interlocking circles, your sister can add other charms (or what we Philly people call medals), while you cannot easily do that with the horseshoe.
I like the one with circles. The horseshoe is nice, but it reminds me of the horseshoe necklace fad after it was on Sex & the City about 12-15 years ago.
If you find something which you like, take the design/designs/ideas to a private jeweler (not a storefront operation) and have them make a nice piece for you.
The work will be of much higher quality, and much lower price. Probably half of the price of the Tiffany pieces which you showed goes for the cache of the little blue box. We've all been there, but learned our lesson. You can learn it early in life and give your sister a piece of heirloom quality which she will treasure for a lifetime.
The horseshoe is less classic and more likely to look dated. The interlocking circles will be able to be worn for longer, because it's not really part of a trend. I have an Elsa Peretti heart, but I don't wear it much, because the chain is shorter than I'd like, and I didn't really want to buy a new Tiffany chain for a present someone else got me.
