Yes there is a sanitary issue. Going without undies in a clingy dress on a hot day may leave a damp spot on the dress. Underwear absorbs moisture.

A seamless "boyshort" would be least likely to leave pantylines, but the slip version mentioned up above will probably work well. Flesh-coloured panties show up the least under white clothing. Flesh-coloured undergarments are even less visible than white under white. Saw that on an episode of Oprah once, bless her.

A G-string or better, commando. Really, as another poster pointed out, there's no danger of anything showing since the skirt is so long. A well-fitted thong or G-string will feel like you are not wearing one, so comfort shouldn't be a concern. Although thongs might show depending on cut.
