If a person creates his own silhouette, meaning picks out a fabric, creates a style, basically designs the item, than this person is a designer. Seamstress is a person who sews. That's it. A designer will provide with the fabric and design, patternmaker will make a pattern, a seamstress will sew.
A knowledgeable designer should be able to make a pattern and sew, because it's important to know construction. Anyone can draw a pretty picture, but not everyone knows the technical part.
what they said...and NO they don't both get the same pay. The designer is usually involved with the marketing (= sales) of the product which is usually where the bulk of their "pay" comes from. The seamstress's job is over after the pattern/patterns have been sewn.

The word seamstress always makes me think of those in the post-elizabethan, pre-victorian English age....... the busty wenches in their tight corsets..... yummy Sigh. I need a time machine.

I think a seamstress sews clothes
and a designer has a sense for fashion, and has the seamstress sew his/her designer fashions they created.
