While I share your disdain for the smelly bare feet on the chair (yuuuck!), I also want to point out that in certain cultures, you're expected to remove your shoes prior to sitting down.
I would have quietly pointed it out to the waitstaff; they're in a position to approach her and remind her that there are health regulations in place. It may have been that the manager was focused on something else and simply didn't notice the smell.

Or, send the chef out to tell her they've run out of feta cheese, and they wish to scrape some from her.
For those who are saying this is about "health codes", why are Japanese restaurants "excluded"?

You will see that even if you are not one to do it, it is not illegal.
Do people actually CARE if someone was wearing flip flops, and took their toes out of the strap when they are sitting? A restaurant would be kicking people out all day every day in the summer.
I find it more offensive when people sit in a booth and put their shoes up on the seat. At least the women had a small courtesy for removing shoes.
Not saying the smell didn't offend anyone.
I owned a small chain of comedy clubs in the 1990s. People wearing flip flops would complain to me that someone took their toes out of their sandals! I'm like, what are you wearing?
