But when you say "I don't like being served" I can't help pointing out that the whole concept of a restaurant is to be served or waited on. If you don't like that, don't go - is will also save you from judging the way other people order their meals.

On a different note - so many people here seem to not like mayo. I'm obsessed with it! Here in England, it's common to dip your chips (fries) in mayo instead of ketchup so I always request it at restaurants. Soooo good. I'll probably get strange looks when I order it in the US, lol.
I'm perfectly fine if the restaurant would put a coffee stand in the corner somewhere so if the servers are busy, I can get up and pour my own. I like it when restaurants use the thermal containers of coffee - more for breakfast than dinner time. Other than that, I don't think I try to make their order taking a night-mare. Most usually know what I want and definitely don't want, unless they run out of something I ask for.

Whenever I order the fries, I like to use malt vinegar and pepper.

Yes. I'm fussy about almost everything.

No mayo and butter on the side. That's about it.

I love malt vinegar on my fries, but fresh ground pepper sounds appealing too.

If I see something on the menu that I find it necessary to change 10 things about, I would just order something else. It is common for me to try to make it at home the way I would have liked it to be.

The only type of salad I will eat at a restaurant is Caesar except at Olive Garden (yes, on occasion I do frequent Olive Garden for soup and salad). On other salads a lot of times the tomatoes aren't ripe, I don't like iceberg lettuce unless it's in a wedge salad with really nice blue cheese, and I don't really like boiled egg, shredded cheese or carrots or onions on my salad.
