I just found that there was an updated cause of death for Joseph, with an addendum to the original death certificate.
Yes, it appears he was in an auto accident. Cause of death here says "concussion of brain .....by overturning of auto in which he was riding on Holme Ave & N.E. Boulevard, Holmesburg."
So those who said it was "automobile case" instead of "automobile cask" were right!

He can, but it would be two different conditions. When a physician says "heart attack" he means muscle damage due to a clogged artery: "myocardial infarction".

But it's not always about clogged arteries. There are also instances of defective heart valves, etc.

A person can die just from the malfunctioning valve. Obstructed or leaky valves wreak havoc on the ability of the heart to function as a pump. The arteries to the heart muscle may be perfectly normal.
