Outside of Smithfield, PA, there's a place called Old Frame, just sounds kind of weird to me.

Although not really a "worst" name, PA has Indiana, PA, California, PA, Jersey Shore, PA.

There's a Washington, PA, but Washington seems kind of common everywhere. But on the interstate signs, I guess not to confuse motorists, it's written as Washington, PA instead of just Washington.
