Do you see it as a physical place or just a state of mind? Also, what regions would count? Personally I'd exclude the 'fringe' part of the US from Middle America. BosWash, New England as a whole, the deeper part of the South, the Desert Southwest, urban California, and the Pacific Northwest, wouldn't count as being Middle America imo.
I would say the entire Midwest, the Rocky Mountain region, the inland Northeast, and the 'shallower' part of the South would be geographic Middle America. But really it is just as much a state of mind.

Midwest and Middle America are two different things.
As far as Middle America goes, I'd argue that the following states all belong: Nebraska, Iowa, Kansas, Oklahoma, Missouri, Illinois, Indiana, Arkansas, Tennessee, Kentucky, and Ohio. All states are inland and middle latitude.
That's if I'm in a generous mood. If not, then cut out Arkansas, Oklahoma, Tennessee, and Kentucky.
When I hear Middle America I think of Central America, so that would be all the countries from Guatemala to Panama. Some also include Mexico but I'd say that is more North than Middle America.
For me, "Middle America" has no specific geographic relevance. It's more of a lifestyle reference; characterized by smaller cities and towns, more conservative political views, and a higher level of conformity to traditional customs.
