This is an odd question, and I basically feel like you're asking about cleanest/dirtiest cities. Most metro regions are made up of suburbs (smaller towns) and smaller cities. I've never been to a dirty suburban town, honestly. Gritty feeling, more urban, sure but dirty? No. I think most metro regions would be considered 'clean' while most cities would be considered 'dirty', with some more so than others depending on their size, density, population, age, etc.

Obviously the richer cities in Orange County. You won't find a patch if dry grass in sight in the rich OC cities and if your car isn't a new model in perfect conition, you're considered "ghetto."
But in a non superficial way, Seattle is very clean considering how dense it is and how many travel to and through it.
Minneapolis and Boston are the cleanest I've been to.
Regarding Boston you need to know where to look for the dirt and grime. It is not on the Freedom Trail, Duck Boats or Fanueil Hall Marketplace. I could show you many spots in Boston that would induce vomiting. I remember coming up from the subway at Downtown Crossing every morning to the overpowering stench of urine. Yes, that street smelled worse than the subway. Boston tries very hard to hide some things from the tourists.
Cleanest BIG city I've been to is Chicago, when I visited NYC thats where I really felt the difference. But NYC is a different animal/city than the rest of the U.S. so its a exception, I would say for its size its not dirty compared with same size cities around the world.
The only major city I've been to that's as dirty as New York is Paris. The rest are much cleaner (especially Tokyo, which is spotless).
Whoever said Minneapolis has my vote.
