It was predicable that Tennessee would be listed multiple times, and yes for good reason. There is a lot of country music roots here, lots of country music and bluegrass music listened to here. I listen to tons of it myself. However country music is popular in every southern state, every Midwestern state and every mountain west state. Country music is common nation wide, but less popular in the places you would expect it not to be. When I have traveled I had a tougher time (though not impossible) finding good country stations in the Northeast states, and very urbanized parts of the west coast. If you live between the Appalachians and the Sierra Nevada/Cascades then country music is popular where you live.
What do you guys think are the most "Country music" cities?
Nashville seems like the clear winner, who are the runners-up? Memphis? OKC? Tulsa? Little Rock?

Memphis has more blues/rock n roll roots. Little Rock is too small and is only 137 miles from Memphis. Oklahoma cities would be second because they already have major country recording artists.

When you think of Country Music, which states come to mind?
Trick question.
Traditional country (whether old or new) is only popular in a few places. Texas is the first place that comes to mind. Real country music still lives in places like Nashville too, but it flies under the radar and doesn't even come close to being played on the radio.
The brand of country that is currently played on the radio is popular in EVERY state. Why? Because it isn't country music. It is slightly twangy pop music. And that is why it is extremely popular. Because it is pop music, not country music.
Generally speaking, the average person does not like real country music.
To me, it seems like Country and Bluegrass originated in the Upper South. States like Virginia, Tennessee, Kentucky, and North Carolina. However, as time went by, it spread out west or to the Deep South. States along the lines of Texas, Missouri, Oklahoma, Mississippi, Alabama, etc.
Country music cities with the exception of Nashville is a little tough. Bristol, TN/VA comes to mind, though. Definitely Tulsa, Oklahoma City, and Dallas/Ft. Worth, too.
