From what I see and hear, both on C-D and in real life, there are really only five "expensive" metros in the entire U.S. as far as housing costs -- New York City, Boston, Washington DC, San Francisco, and Los Angeles.

Granted, there are more expensive and less expensive places for housing in each of these five areas, but on the whole, they're considered the five places people do NOT consider when they ask for somewhere "affordable" to live. So is this list right? Is every big metro in the U.S. outside of these a place where most people can afford a middle-class standard of housing (either apartments or houses) without too much of a strain, or having a really high-paying job? Or are there other areas besides these five that you'd include?

Not really. Those areas aren't as expensive as they could be. NYC and San Fran are the super expensive areas.

The cost of housing and rentals in Denver has really skyrocketed over the last two years.

There are expensive areas everywhere. Take Aspen, CO, the entire state of HI, etc.
