What are some metro areas that you know of that have less than 500,000 people and that have at least two of the following; an opera, orchestra, live theater and/or ballet?
Asheville NC, has both a ballet and a symphony orchestra, though I've never been to either. Presume that they offer only part-time seasons.
Also the area in the Berkshires is probably under 500,000 and has tons of theater, opera, classical music in Pittsfield, Great Barrington, Williamstown, and probably a few other places nearby. Again, I'd guess that most of these offerings are dudring summer tourist season, but not all.
Colorado Springs has a local Philharmonic that regularly performs both in The Pikes Peak Center as well as throughout the city during summer months. There also was a recently opening of a large performing arts center with several stages, regular theater groups as well as the ability to host traveling shows. City population is around 450k.
Williamsburg has an opera company, a few different theatrical groups, and an orchestra. Ballet troops visit for performances; we just saw The Nutcracker here, performed by a troop from Richmond. Population is 15,031.
Mountain cities tend to punch above their weight in these types of rankings.
Ashland, Ore. and Staunton, Va. are both known for Shakespearean theater. The latter (population <25,000) is near Charlottesville, which has a professional ballet and orchestra, and also has an annual chamber music festival.
Rochester, New York has Geva Theatre for performing arts and the Eastman Theatre plays host to the Rochester Philharmonic Orchestra. 75 miles to the west-southwest, Buffalo has live performing arts at the historic Shea's Theatre and nearby Kleinhan's Music Hall is home to the Buffalo Philharmonic Orchestra.
