I have spent the last few years trying to narrow down where to go. Looking for a spot to buy a home on 10 or more acres in a forested setting. Budget will likely be $400,000 range. I want rueal lifestyle with decent hospital and work not more than 30 minutes from where I live. Is that possible? I was leaning towards Tennessee or Virgina but would consider a little more north like PA now. I am in central MT and have had it with the high wind and the smoke in the summer from all the forest fires are destroying my lungs.

Would help to give a preferred community size range to be near.

100k, more, less (25-50k?), way less (10-25k), pretty small (5-10k)...

What do you think about checking Lewistown PA, South of Morgantown WV, Lexington VA, Wytheville VA, Toccoa GA, Valhalla SC, Greeneville TN...?

Those are all areas on my radar and I plan on taking a trip that way in a few months to check out as many as I can as long as gas doesn't get higher yet. As far as size of town/city I don't really want to live right in town because I want 15 or more acres but close access to a town with good medical a decent job is all I need so anything from 15K to 200,000 could fit the bill. I would prefer to be no more than a 20 minute drive. I see some nice stuff in upstate NY but the property taxes are mind bogglingly high there. My wife is from Va but we mainly want a climate that still gets winter and isn't too humid/hot in summer.

You've had some good suggestions on your other threads. I think at this point instead of asking basically the same question again maybe start visiting some of the state forums to get a better idea of the pros and cons of those areas. (and as you've seen don't mention two states in the same thread on a state forum, because, well just because)
There are some great tools to research some of your wants also. Such as using weather history and dew point comparisons to get an idea of places that meet your climate needs. Because when you say things like "isn't too hot or humid" you're going to get many different answers and they may or may not fit your own idea of what those conditions are.

I get that. I did post in a state forum and it got kicked right into here by a mod. I am going to spend about 14 days exploring in a few months and that should help.

Something to keep in mind with upstate NY is that the property taxes you see aren’t necessarily what you pay due to not factoring in property tax exemptions: https://www.tax.ny.gov/pit/property/exemption/index.htm

You also have to consider pay, which is likely higher than the other places you are looking into. A good source for viewing that information: https://www.bls.gov/oes/current/oessrcma.htm

There is also this Housing Opportunity Index, which uses median family income and median home sales price to show the percentage of homes one can buy in that area: https://www.nahb.org/-/media/NAHB/ne...nk-2021q3.xlsx (Complete Listing by Affordability Rank: https://www.nahb.org/News%20and%20Ec...ing%20criteria ) This considers property taxes as well.

So, those are some things to consider when looking, as they may not be so obvious during the search.

With that said, there are areas of WV, PA, MD and VA high on the Housing Opportunity Index list as well.

Given the OP's latest information PA and WV make the most sense in my opinion with milder summers (once outside the urban heat islands like Philly). Check out rural areas around Hershey and Penn State University Medical Center's facility there. https://www.pennstatehealth.org/loca...medical-center

^A sleeper area of PA that would work in terms of a health care system is around Sayre, Athens, Towanda, etc. near NY(Waverly, Elmira, Corning, Owego, Binghamton, etc.). They have the Guthrie Health System on both sides of the Twin Tiers: https://www.guthrie.org/
