To me, it depends...
The Miami most people think of and visit (Downtown, Brickell, Miami Beach) does not feel foreign at all. It just feels like "Florida."
Miami West of I-95 can feel foreign.
Which towns by Lake O are you referring to?
I agree that the Americanness of SF is understated here, but in a discussion about the most foreign feeling place (in the lower 48 anyway)...
East of 95 that isn't predominantly Latino still contains areas like Sunny Isles and Aventura where most people are from another country and you hear other languages as commonly as you hear English. Miami Beach will have a lot of American tourists but, again, most residents are immigrants.
I mean when I go to Miami I know I’m in America but both places are similar as far as feeling foreign. A true American who is only interested in “American” cultured stuff would probably have a hard time fitting in NY. Probably wouldn’t even fit ok well with the Italian Americans either
If you mean really rednecky kind of American stuff that you'd find in the rural South then yeah but you could easily live a very culturally American life in New York. Think of all the American restaurants (including chains), 2 basketball teams, two baseball teams, two hockey teams, English being the dominant language, probably the most visited stop for American musicians, comedians, plays, etc.
Ironically many of the Italian-Americans I know are some of the most stereotypically American people, the type to have bald eagles as their FB profile pic.
It’s not all of Miami, but it’s a lot. I’ve been surprised how much I’ve needed to use Spanish there and my Spanish is rough. haha
Yes, I agree. I'd say it's MOST of Miami. The vast majority of the city/county is West of I-95. I was more referring to the areas outsiders are mostly familiar with, which is a small segment, but the main one that comes to mind to most.

Miami posters here don't want to be associated with Florida or the south.
Hawaii has a built-in unfair advantage due to its isolated geography in the middle of the Pacific Ocean thousands of miles away from the Continental US. It better feel foreign being as remote as it is.
If we're talking about mainland US, then I would say Miami (maybe, although I never felt like that down there when visiting, even when in Little Havana) or somewhere in South Texas by Ciudad Juarez or one of those border towns.
