Interesting, you actually got me to take out a map to figure it out:

Farthest in the Continental US:

North - Blaine, WA on the way up to Vancouver.
South - Key West, FL
East - Somewhere in Maine on the way to Newfoundland. Probably Houlton, ME.
West - Portland, OR

Farthest Internationally:

North - Berlin, Germany
South - Singapore, Singapore
East - Tokyo, Japan
West - Honolulu, HI

Yep, I've never been extremely far north or anywhere in the Southern Hemisphere!

North: Amsterdam, Netherlands
South: Koh Rong, Cambodia
West: Seattle, USA
East: Tokyo, Japan

The number of states I've traveled to in the US is disproportionately smaller than the number of countries I've visited, so I'm going to leave that out

Looking at my old post, and comparing to google earth I corrected it, as I made some major errors.
Furthest north, London England
Furthest south, Everglades Florida
furthest east, Innsbruck Austria
Furthest west, San Fransisco California

How about another two points: closest to the Equator and closest to the Prime meridian?

In my case that would be Singapore (1.2° N) and Greenwich, UK (0°).
Closest to equator ? Equator itself
Closest to prime meridian ? My westernmost / Padang 100E
Closest to timezone meridian ? Soekarno-Hatta airport (106E/UTC+7) / land, Lampung (105E/UTC+7) / air (in flight)
