People from smaller towns say block too, it just depends on if you're "in town" (where there are actual blocks) or outside of town.

I live on US 31 which starts in Alabama and goes all the way to Mackinaw City, MI... so if I'm giving directions from home I may say "it's up the highway about 5 miles".

Not just bigger cities use the term block. I use all of the above; however, I use street the most.

I mostly say street but when I'm in an inner city I say blocks because they are actually blocks. Or if I'm giving directions I'd say 'pass two blocks' because I think 'pass twoo streets' sound awkward to me lol. I will say 'go down this street.' I never used road.

Depends on where I am and what or how far we're talking about.

If we're in town and talking about how far something is and it's really close I'll say, "It's just down the block from here." Or if we're talking about something in another part of town I'll say, "It's just down the street from that Balkan restaurant we went to yesterday." If we're in a rural area outside of town or in the suburbs where there are long, ungridded roads (as opposed to city streets) I'll say, "It's down the road a little further and it will be the 2nd driveway on the right after you pass Koehler Road."

When in town I say block, when at my place I say road just because there are more of them then blocks.

What the hell is a "block"? I've never seen street patterns that would suggest this "block" people keep mentioning.

Are you trying to tell me there are streets out there that don't curve repeatedly before leading you to a dead end or one way heading in the opposite direction?

Living in northern NJ we often say highway more than all of the above.
The Garden State Parkway vertically cuts through the center of my city. You either live east or west of the highway in this city. (Though the cool ones reside on the east side )

Nah, but my mom, the new yorker that she is, says block. I say Street.
"Up the street" as opposed to "Up the block"
