I have a little one starting Kindergarten this August and we just moved to a new county. I took her to the doctor on Friday for what I thought was her Kindergarten appt....shots, etc. Well now I am looking and it says that Kindergarteners are required to have a Eye, Ear, Dental Record (Form 3300) and the doctor didn't do that and I do not have that form. Is this a simple eye and hearing test that they do? Can anyone give me any info about this? Why didn't they do this on Friday for me? Now I am going to have to make another appt at the doctor. What a pain! Sorry to vent!

Hi - here's some info based on our experience. I would contact your doctor - you shouldn't need an addt'l appt if she just had her checkup. They have the forms on hand and should be able to complete and sign for you. You will also need to get a dentist to the do the dental portion, unless your doctor is able to do it. Hope that helps.

you can get that form at the health dept, and they'll do all the tests.

They ask your kid to look at some pictures and ask questions about it, and for the hearing test they put headphones on them and check their thresholds.

It only takes a few minutes.

Yes, I agree with southgeorgia. When we first moved here and didn't have a pediatrician we took our kids to the health dept. It was really cheap and quick. Got the forms and the problem was solved. Just check into your local health dept.

The doctor said to just stop back in. I just hope we don't have to wait forever since this time we won't have an appointment and will be technically a "walk-in". I hope I do not have to take her to the dentist and they can just do the dental part at the doctor but we will see. Thanks for your help.

I agree that the child's doctor should have done it for the Kindergarten appointment. I had the same thing happen to me! It is a pain - I have to miss work and take my daughter out of her activities to get the screening.

So why is it necessary to have daycare children take this test. My childs daycare is asking that the kids there take the test. Some doctors states it's not necessary the childern are between 2 & 3 years old. Marietta, GA.
