Or are you suggesting that strokes are a big problem in Winston, but not so much of a problem in Greensboro, Charlotte, Raleigh, Wilmington, Durham, High Point, etc.?

Perhaps some facts would help. NC is not among the worst states for stroke incidence. Not among the best, but certainly not the worst:

There was also county level data in the link I posted. Strokes are pretty well distributed throughout the southeast.

Also, since WF Baptist Medical Center is a major teaching hospital, as well as one for patients, it makes sense that a stroke treatment center would be located there, considering that stroke is a major cause of death and disability.

I grew up in Salisbury. Many of my parents' friends went to Winston all the time for their health care. Those hospitals draw from a wide area, not just WS.
