The requirements for grad school admissions are for the most part set by departments; they're not set by a central admissions committee. (The exception is something like a minimal TOEFL score for foreign students, to be sure they can work as TAs and learn in an English classroom.) You'll need to check the requirements with whatever department you're applying to.

I'll say that I'm on the graduate admissions committee for my department. We have the GRE listed as recommended but not required. I always find it questionable when someone has opted not to take the GRE, and will probably downgrade the application slightly. I will also say that if you're afraid of a standardized test, grad school may not be a great choice for you. The GREs are a cakewalk compared to graduate classes, qualifying exams, orals, etc.

Also this coming February 2011, is the last time the GRE test will be the same as they will be changing it up for the next round of test come Nov.
