I just received my second survey (duplicate of the 1st one) from CMS. It says in the cover letter that I was "selected at random."

The purpose of the 8 page survey is to tell CMS about the quality of care I am receiving. The letter also states that the information in the survey will not be shared with anyone "other than authorized persons at CMS." And they insist they will not share the completed survey with any healthcare providers.

The survey has the name of my PCP printed on it and 20 of the questions are about that doctor.

There is a barcode at the bottom of the first page, and another bar code at the bottom of each page, so those are places where my name is likely included for them to retrieve.

If it was truly anonymous, they wouldn't know whether I returned the first one or not.

So, I'm wondering if anyone else has received a survey like this.

There are 20 questions about the specific PCP, 2 questions about the clerical staff at the provider's clinic, 3 questions about specialists (no names), 7 questions about overall care, and 25 questions about me (demographics, whether I need help with everyday tasks, do I have internet, etc.)

My hesitation about filling this out is having the name of the PCP printed on the survey. I've not seen that before in any survey where a specific individual is singled out and named. Nor have I ever received a survey from CMS before (6 years on Medicare).

Anyone else receive this kind of survey from Medicare (CMS)?
