My vote is for #1, but #'s 2 and 3 are good too. I also agree that having a light above the fan blades (4 and 5) produces a strobe effect. If not on the room surfaces but also what you notice from the corner of your eye when the light is repeatedly and rapidly blocked from view.
Definitely #2 because it's more traditional in style. Agree with other posters that it depends on your decor. If you're thinking resale, go with the consensus.
Personally, #2 because it seems to be a more "transitional" in style. I never use the light fixture on my ceiling fans. Honestly, I have not seen many ceiling fan light fixtures that I find attractive.
When choosing a fan with a light, remember that if the light shines UP through the fan blades, you'll notice the effect on the ceiling, especially on low speeds. I don't generally like that (it looks like a strobe) and the light on fan 3 looks best to avoid it.
I agree with Gary. The lighting disturbance from the typical fan light design, I despise as well. I'd go with #3.
I love #2! #1 would be my next choice followed by #3. Don't really care for 4 and 5.
