I have a 1991 Honda Accord EX, I can not move the gear shift from Park to Drive or reverse unless I insert the ignition key in the Shift Lock Release. Is this a major repair or a minor one?

I had something similar happen with my '99 Acura, I seem to recall the repair set me back about $500 or so.

1. shift position sensor on the gear selector on the floor.
2. any power going to shift-interlock solenoid
3. the brake switch (do the brake lights work, even the 3rd light)

you can look for a blown fuse .Check fuse #1 (10amp) in under-dash fuse panel and fuse #25 (20 amp) in under-hood fuse panel. Also see if brake lights are working and horn is working. If they are not, fuse #25 is blown.
I believe the # for the fuses should be the same for your car, reference your owners manual or look for a reference on the fuse panel cover.

I just called the dealer, they said the problem is probably a brake light switch.
