If the surface is lap-board Hardi, I required my cornice guy to affix them with zinc coated screws into studs. The screwhead would be counter-sunk just slightly in order for the painters to fill and paint. No need to caulk the perimeter because it would inevitably trap moisture behind it. The trim that wraps the actual opening of the garage door should have drip-edge, the decorative header would be mounted about a 1/2" above the drip-edge.
With brick I spec'd two Tapcons and construction adhesive; stucco was just construction adhesive (no penetrations).
