In my location, a permit was needed just because part of the work extended into the road right-of-way (just as all driveways do). They didn't care about what was done beyond the right-of-way. The permit was something like $25-50, the minimum. We just submitted a simple drawing of the addition.

As for how big, you need to check with your city. They may have restrictions on how wide your driveway can be at the road. I'd guess there are no restrictions once you're out of the right-of-way. You could probably pave your whole lot.

Usually concrete flatwork on your property does not require a permit.
