About a $1000 with a stone shelf- figure about $850 w/o stone shelf.
That's for real stacked stone. You might be able to knock a couple hundred off if you go with cultured stone.
the shelf is the mantle - adding a hearth would add some money. The cost of the project depends on the type of stone you use. This is a link to a stone mason contractor in your area. They could give you the best estimate, but do your research, I found this as a result of a google search of NC masons.

I was referencing to the pictures.
The stone surround had a wood-trimmed "shelf/mantle".
I prefer a stone "shelf/mantle"- it has a more earthy/rustic feel to it.
