Have people who have moved from homes in Houston to Sugar Land, for instance, seen significant increases or decreases in their water and sewer rates? In addition, are rates set at a county or city level?

It's all over the board depending upon where you are talking about. Some of the newly-developed (last 5 - 7 years) 'burbs are always going to be higher than 'hoods developed for decades. As to where they are set, County, MUD, City .. again, it depends upon where you are talking about. It can be one, two or all three.

Ours went down both times moving out. But, we're also not living in a brand new MUD.

Though I hear everything is going to go up like crazy with this surface water business.

My MUD rate is much lower than city rate but of course when you add on the tax it is higher.
