A roach could of slipped by and fell into his soup and I would see how the restaurant reacts before filing a complaint.
The truth, as someone who knows quite a few thing about food industry. A lot of times, roaches can be picked up everywhere from farms to restaurants. Roaches, or all kinds of animals can be picked up by 18 wheelers who parks overnight at a truck stop surrounded by trees. Or the supplier gets bug from opened dock doors and of course, restaurants gets it from unsanitize kitchen. What I'm trying to say roaches can come from different places, not just restaurants. Not all fruits and vegetables are shipped in enclosed 18 wheeler trailer box, like yellow onions and green cabbages which sometimes delivered in an open trailer with just a cover on top of it. I've seen a snake in a case of cucumbers at a food supplier warehouse once in a photo, of course all of it was thrown away. When food supply gets deliver to restaurants, there is no way you can deliver without opening the door in which flies and other things can get in. My coffee a few days ago had some kind of flying bug in it, if I didn't stare at the bug fly into my coffee. I would of thought someone tampered with my coffee, or blame on bad coffee packaging.

If the OP called the Health Dept., they will send inspector to check it out for sure. They can tell if it is an isolated incident, or bad sanitations on the restaurant. And they will check on how many complaints are on file on that restaurant to, if there is consistency on record and unsanitation, they can close the restaurant for a day, or weeks for them to do extermination. Don't go out to eat if we are worry about roaches.

If we knew the amount of rat poop or ground up bugs that are permitted in peanut butter, flour, corn meal and other food, we probably would stop eating anything not prepared from scratch.

The problem with the roach in the soup is that no one at the restaurant saw it. Was it in the soup as it was being prepared or did it come in the container or just drop in the container out of the sky?

The presence of a roach in served food demonstrates a lack of care. If the help does not bother to look at served food, what's up with what we don't see?
