They will use existing medium that you have in your home usually (some may install as required). If you have cable ran through your home, the TV service will use this.
As for DSL, if it is ADSL then likely it will be using your existing phone line and little if any changes needed. They will simply use the extra unused pair in your phone wiring and you will be given DSL filter to use for analog devices you have so it doesn't interfere with the DSL.
